1. Acceptance of Terms
By accessing the AVIVA MedTrans LLC website at avivamedtrans.com or booking our non-emergency medical transportation (NEMT) services, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our website or services.
2. Services Provided
AVIVA MedTrans LLC provides non-emergency medical transportation services including ambulatory, wheelchair, stretcher/gurney, and long-distance medical transport throughout Miami-Dade, Broward, and Palm Beach counties in Florida. We are a licensed NEMT provider and do not provide emergency medical services or ambulance transport.
Our services are subject to availability. We reserve the right to refuse service at our discretion in accordance with applicable law and safety requirements.
3. Booking and Scheduling
- Rides may be booked online, by phone at (844) 201-0110, or by email at info@avivamedtrans.com.
- We recommend booking at least 24–48 hours in advance. Same-day bookings are accepted based on availability.
- You are responsible for providing accurate pickup/destination addresses, appointment times, and patient information at the time of booking.
- AVIVA MedTrans is not liable for missed appointments resulting from inaccurate information provided at booking.
4. Cancellations and No-Shows
We request at least 2 hours notice for cancellations or rescheduling. Repeated no-shows or last-minute cancellations may affect your ability to book future rides. For Medicaid-authorized rides, cancellation policies of the relevant managed care plan or broker also apply.
5. Payment and Insurance
- Payment terms depend on your coverage type: Medicaid/Medicare, private insurance, or private pay.
- It is your responsibility to verify insurance authorization prior to your ride. AVIVA MedTrans is not responsible for denied claims resulting from incomplete or incorrect insurance information.
- Private-pay rates are provided at time of booking. Rates are subject to change with notice.
- Invoices are due upon receipt unless a billing arrangement has been established in advance.
6. Patient Conduct and Safety
Passengers and accompanying individuals are expected to behave respectfully toward AVIVA MedTrans staff at all times. We reserve the right to terminate a trip and refuse future service to any individual who is verbally or physically abusive, under the influence of illegal substances, or otherwise poses a safety risk.
For safety reasons, all passengers must comply with vehicle safety protocols including seatbelt use and proper securement of mobility devices.
7. Limitation of Liability
AVIVA MedTrans LLC is not liable for delays caused by traffic, weather, mechanical issues, or other circumstances beyond our reasonable control. We are not responsible for missed appointments, medical outcomes, or consequential damages arising from transportation delays.
To the fullest extent permitted by Florida law, our total liability for any claim arising from our services shall not exceed the amount paid for the specific trip giving rise to the claim.
8. Website Use
The content on avivamedtrans.com is for general informational purposes only. We make no guarantees regarding the accuracy, completeness, or timeliness of website content. Unauthorized reproduction, distribution, or use of our website content is prohibited.
9. Governing Law
These Terms of Service are governed by the laws of the State of Florida, without regard to conflict-of-law principles. Any disputes arising under these terms shall be resolved in the courts of Miami-Dade County, Florida.
10. Changes to These Terms
We reserve the right to modify these Terms of Service at any time. Changes are effective upon posting to this page. Your continued use of our services after changes are posted constitutes acceptance of the updated terms.
